Film Request Form

Many of the DVDs and videos in the Lee Library are purchased because of a faculty request. Often times a requested title can be sent to the library for a thirty day preview. You and the subject librarian responsible for your department will be notified when a preview title arrives so it can be reviewed and recommended for purchase or return. Title requests that are not available for preview will be sent directly to your subject librarian for purchase review. If purchased, the title will be housed and circulated by the LRC. Requests should be items that the BYU libraries do not currently hold and would enhance teaching and research efforts. (Fields below marked with an * must not be left blank).
